PRELIMINARY STUDY ON THE EVALUATION SCHEME OF PRIMARY CARE ROLES OF PRIMARY HEALTH CARE CENTERS

Abstract
The primary health care service in primary health care center rests major area to resolve -the rapidly increasing health care demand and problems of supply in the health care¢¥ delivery system. Therefore, the strengthening of primary health care role and the evaluation of the program should be needed. In this study, a schematic model for evaluation of primary health care role consisted with 2 phases; `Assessment¢¥ and ¢¥Feed-back¢¥ phase, and its measuring instrument was developed and tested for the practical utilization.
The measuring instrument was consisted with 47 questions; 28 of those were requested by health care consumers and 17 were from physician¢¥s judgments. For assessing the program, in the first phase of the model, the Primary Health Care Index was calculated, but also the scores classified by five essential elements of primary care such as accessibility, accountability, comprehensiveness, continuity and co-ordination.
In feedback phase, data obtained were recategorized into management variables¢¥ such as `Utility and efficiency¢¥, Model of primary care¢¥, Medical doctor performance¢¥, `Medical record keeping¢¥, `Referral¢¥, `Paraprofessionals¢¥ et al, to feed back for improvement of future program.
Twelve physicians and 362 registered families from one of primary health care model clinic, the Family Practice Model Clinic of Seoul National University Hospital were selected -to examine the reliability and applicability.
The following conclusions were obtained;
1. On the test for the measuring instrument
1) The reliability of the instrument, by comparison of the variance value between subgroups by socio-demographic variables, was proven to be significant except on the accountability.
2) On the consistency between items for each element of primary care, Cronbach¢¥s Alpha showed to be higher as above 0.7, but on accountability and comprehensiveness, the adjustment seemed to be necessary.
2. Using this scheme, the assessment scores of FPMC in SNUH, the scores of the 5 primary care elements were calculated in the range of sixty to eighty points, so it showed that primary health care was adequately served in this model clinic.
3. On analyzing by management variables in the FPMC, some problems to be improved for future program was shown by the items with lower score and open questions.
